Aged care is one of six key priorities for Primary Health Networks across Australia and an important area of focus for Country SA PHN.
CSAPHN works closely with and supports general practitioners, allied health professionals, and the Country Health SA Local Health Network and others to improve the availability of health services for our ageing population.
Current CSAPHN programs support older people with age-related health issues including Chronic Disease Management, Disease Screening and Prevention, Mental Health & AOD.
