All Primary Health Networks (PHNs) were required to develop work plans for the financial years 2016-2018 and submit these for approval to the Department of Health. The plans outline which activities the PHNs propose to implement in order to achieve the key objectives of enhancing the effectiveness of medical services and improving the coordination of health care. Country SA PHN’s work plan details a broad range of activities that reflect the particular needs and priorities of Country South Australians identified in our annual needs assessments. The activities outlined in this document are presented by funding stream, a general funding stream (PHN core funding) and a funding stream specifically for after hours health services.
